Hypertension in Pregnancy

Hypertensive disorders of pregnancy are classified by when the blood pressure rose (before or after 20 weeks) and whether proteinuria or end-organ dysfunction is present: chronic, gestational, preeclampsia–eclampsia, and superimposed preeclampsia. This lesson covers the criteria, the workup, pregnancy-safe antihypertensives and the fetotoxic ones, magnesium sulfate, and when to deliver.

Preeclampsia and Eclampsia

Preeclampsia is new hypertension at ≥20 weeks with proteinuria or end-organ dysfunction; eclampsia adds seizures and HELLP adds hemolysis, liver injury and thrombocytopenia. This lesson covers the placental pathophysiology, the US risk and aspirin criteria, severe features, the differential, magnesium sulfate and its toxicity, severe-BP control, delivery timing and prevention.

Acute Fatty Liver of Pregnancy

Intrahepatic Cholestasis of Pregnancy (ICP)

Intrahepatic cholestasis of pregnancy is a late-pregnancy liver disorder with intense itching of the palms and soles, no rash, and serum bile acids 10 μmol/L. The mother does well, but the fetus is at risk of sudden stillbirth, meconium-stained fluid and preterm birth. This lesson covers the diagnosis, ursodeoxycholic acid, fetal surveillance and planned delivery at 36–39 weeks.

Thyroid Disease in Pregnancy

Thyroid disease in pregnancy is mainly hypothyroidism (Hashimoto thyroiditis or iodine deficiency) and Graves hyperthyroidism. Estrogen-driven TBG and first-trimester hCG change the test results, so diagnosis relies on TSH and free T4 read against trimester-specific ranges. The lesson covers the levothyroxine dose increase, the PTU-to-methimazole switch, TRAb and fetal risks, and thyroid storm.

Headache in Pregnancy

UTI in Pregnancy

UTIs in pregnancy range from asymptomatic bacteriuria to acute cystitis and pyelonephritis, and all bacteriuria in pregnancy is treated. This lesson covers why pregnancy predisposes to infection, screening and diagnosis, antibiotic choice by gestational age, and inpatient management of pyelonephritis. A master table compares the three conditions.
